Here you can request a Wheelchair Foundation Information Packet, which will be sent to you in the mail. Please fill out the form, using the address to which you would like the information packet shipped. This request will be emailed to us, and we will get your information packet out to you.
NOTE: International shipping can be expensive, so we are not always able to fulfill requests from countries outside the USA.
